Plan Details
This plan is unique in that it provides the following:
- Family deductible is limited to three times the individual, keeping out-of-pocket costs low.
- Inside the U.S., choose a provider from the Preferred Provider Organization, or PPO, to save additional out-of-pocket expenses.
- Choose the deductible from a number of options: $150, $250m $500, $1,000, $2,500, or $5,000.
- Maternity and newborn care (including $500 for midwife services) is covered as any other illness after 10 continuous months of coverage.
- Preventative care is available (after 12 months coverage) and is not subject to the deductible (limits apply).
- Hospital indemnity up to $100 per night is included.
- Mental health disorders are covered (after 12 months of coverage) up to $25,000 (lifetime maximum).
- Emergency reunion is covered up to $15,000 if the insured is hospitalized.
- Vision is included (after 12 months of coverage) for routine eye exams, corrective lenses, and more (limits apply).
- Term life and AD&D is automatically included for groups of 10 or fewer employees. For larger groups, term life coverage is optional.
- Outpatient prescription drug card is issued for each covered person and the employer can choose from three options for prescription benefits.
- Dental benefits are also offered in three benefit levels and may include preventative and diagnostic, major dental, and orthodontia.
- Plan comes with assistance services that include pre-trip health advisories, emergency travel services, lost luggage tracking, emergency cash transfers and more.
- Employees can access plan information by visiting an online service for account management and resources like pre-certification, ID cards, locating providers and more.
Limitations
The following are the limitations on this plan’s coverage:
- Plan includes coinsurance (plan pays 80%) for out-of-network medical care inside the U.S. up to the first $5,000.
